No, excepted words remain unchanged, not become lowercased (this is not obvious, but this is how plugin works).

You can make 2 or more steps (without writing changes to tags) by changing rules and clicking 'Reapply rules' button. For instance, to achieve desirable titlecasing for sentence 'a bad attempt in putting on a smile':
1. Sentencecase a, in, on, etc.
2. Titlecase all words except for a, in, on, etc.
You'll get 'A bad attempt in putting on a smile' on the 1st step and 'A Bad Attempt in Putting on a Smile' on the 2nd.

Any chance you might be willing and able to add an option 'always capitalize first and last word'?
That would make it near perfect with less steps needed.

Ah, that confused me too.
Maybe a second line of entry could be added to the panel so you'll have:

1. Except for words:  . . . . .
This will apply casing rules on those words, so e.g. 'A' will become 'a' when 'Title Case' is set with the words "a, an, the, in, etc." as exceptions.
 
2. Exclude the words:  . . . . .
This will exclude those words for any change at all. So UB40 will stay UB40, etc.
