How do I make the four options default for everything?

Set it up on tags (1) preferences

quite clearly you need to tick the last tick-box for "cover.jpg"

I did but it won't save anything to the album folder

Make sure you tick the bottom option in "save artwork to" dialog as well when saving.

Looks like the option in the dialog doesn't change in accordance with the tags(1) setting. Every time the user has to tick it manually.

When replacing an existing picture, MB will preserve the existing settings for that picture. The Tags(1) artwork storage settings only apply when a new picture is added.
