If you (or auto-organise) renames a file, MB should be renaming it in the history so that one record is maintained.
But if you convert files, or copy them in other ways, that could explain why the same artist/title is appearing multiple times.

That's strange. I have no recollection of having deleted it. Can I recover it? Or is it just a normal auto-playlist, in which case, can you please screenshot the settings for yours so that I may replicate it?

I don't use it at all, but here is a link to the stock file "Recently Played.xautopf" created by MB, you should be able to drop it into your MusicBee/Library/Playlists folder and examine/edit in the normal playlists GUI.  SHA-1 of the zip should be 2D795E1DB4F0D5F315E4A51E73BFDD8F09F03867 :  https://filebin.net/ysynkhfned6o2ajj/Recently_Played.zip?t=p7upq6z2

Awesome, thanks! I ended up grabbing one from a previous install of the portable version I had and its settings are different from yours, but I've edited it to suit my preference.
