I synced to my phone then noticed a song had the wrong rating so I fixed it and synced again. On the second sync it said 4 files no longer existed in the library and asked if I wanted to remove them from my device. After I let the sync complete I looked in the library and those 4 files are in the library. I also made sure that the files were not re-added to the device after being removed.

I have had this issue a few times with various files. There does not seem to be a pattern to this issue. Syncing twice in a row should not have two separate results.

unless you can provide more information into the circumstances such that i could reproduce the issue then i wont be able to help

This happens to me sometimes, but I think it's because tags changed such that the filename would change, and MB hadn't quite got everything sorted out the first time.
